Registration Information
- Participants may register for the complete three-symposium series
or for individual symposia.
- Registration fee includes dinner and refreshments.
- Payment must accompany registration form as registration is not confirmed
unless payment is received. (No fax registrations accepted)
- Registration will be limited to the first 100 applicants.
- Registered participants will be sent a confirmation letter with
travel information and details of the schedule and location of
the program.
- Refunds for an individual symposium, requested no later than one
full week before the symposium, will be made in full or prorated
for the series registration.
